Bridging the Gap Pilot Program
Pennyrile Area Development District was selected by the Department for Aging and Independent Living to participate in the Bridging the Gap pilot program. This is an evidence-based pilot project with the aim to provide short-term assistance that supports a person with a dementia diagnosis and their caregiver for up to six months while applying for Home and Community Based waiver services. The goal of the Bridging the Gap program is to provide assistance and support to improve the overall well-being of the individual with dementia and reduce the caregiver’s stress and burden.
