Pennyrile Region Needs Assessment Survey

The Pennyrile Health & Family Services Department is conducting a regional Needs Assessment to better understand the needs of older adults and individuals with disabilities across Caldwell, Christian, Crittenden, Hopkins, Livingston, Lyon, Muhlenberg, Todd, and Trigg counties. The information gathered will guide future planning and help demonstrate the need for essential programs and services in our communities.

We invite you to take a few minutes to complete the survey using the link below. All responses are anonymous, and the survey will remain open through December 5, 2025.
