Medicaid Waiver Waitlist Letters

Dear Participant, Parent, Guardian, or Caregiver:

The Kentucky Department for Medicaid Services recently mailed an important letter titled “Action Required - Medicaid Waiver Waitlist Verification” to individuals on the Home and Community Based (HCB) Waiver waiting list.

This letter is legitimate and is not junk mail. Please do not discard it.

To remain on the HCB Waiver waiting list, you must:

1. Take the Provider Attestation Form included on pages 3 and 4 of the packet to a qualified provider.
2. Have the provider complete and sign the form. Qualified providers include:
   1. oLicensed physician
   2. oLicensed physician assistant
   3. oAdvanced practice registered nurse
   4. oLicensed psychologist
3. Return the completed form so it is received no later than January 31, 2027, at:

If the required documentation is not received by the deadline, the individual’s name will be removed from the HCB Waiver waiting list. Because completing the form may require an appointment with a qualified provider, families are encouraged to begin the process as soon as possible.

Submitting the form does not guarantee eligibility for waiver services. It allows the Department for Medicaid Services to review updated medical information as part of the waiting-list verification process.
