Jill Collins

Director of Health and Family Services
As the Director of Health & Family Services, Jill oversees the overall strategic, operational, and administrative functions of the department, ensuring the delivery of high-quality programs and services that promote the well-being, independence, and quality of life of older adults, caregivers, and persons with disabilities within the service area. Jill manages staff, resources, and community partnerships to comply with federal, state, and local regulations while advancing the department’s mission.  Jill is responsible for program development, community engagement and partnerships, and compliance and reporting to funders, stakeholders, and governing bodies.

Amy Baumhofer

Social Services Case Manager
Amy provides case management to clients enrolled in the Participant Directed Services Program/Home and Community Based Waiver.  She meets with clients in their homes to conduct program visits and training.  She provides case management on a monthly basis, coordinates services and provides linkage to community resources.

Sylvia Chase

Social Services Case Manager
Sylvia provides case management services to clients enrolled in the Homecare and Older Americans Act programs.  She conducts assessments for elderly and disabled individuals who are seeking in home support services, ensures access to the continuum of care, ensures appropriate services are provided, ensures the changing needs of the clients and their caregivers are addressed, assures that high quality services are provided in a timely and cost effective manner, and refers clients to appropriate formal/informal agencies to help address any unmet needs.  Sylvia covers Hopkins County.

Jessica Davis

Veteran Directed Care Support Specialist 
As the Veteran Directed Care Support Specialist, Jessica assists with  organizing and facilitating team meetings, trainings, provides administrative and operational support to the Veteran Directed Care team. She coordinates communications between internal staff, the Veterans Administration Medical Centers and partnering agencies. She responds to inquiries from veterans, caregiver and stakeholders who need assistance with program policy. She is also a point of contact to assist with new referrals, program data and internal reports/audits.

Kristin Frazier

Social Services Case Manager
Kristin provides case management to clients enrolled in the Home and Community Based Waiver Program.  She meets with clients in their homes to conduct program visits and training.  She provides case management on a monthly basis, coordinates services and provides linkage to community resources.

Angela Gore

Administrative Assistant
As Administrative Assistant, Angela provides essential administrative and clerical support to the Area Agency on Aging and Independent Living  (AAAIL) programs. Her responsibilities include managing correspondence, scheduling meetings, maintaining records and databases, assisting with reports and program documentation, and supporting program staff with day-to-day operations. Angela provides detailed review and maintains all in home service client files. She also provides correspondence with all advisory council, service providers, and agency partners.

Ali Jones

Aging & Disability Resource Center Coordinator
Ali is the primary contact for all referrals, intake and eligibility pre screenings for clients accessing services. She also maintains a regional resource directory and assists with managing the waiting list for services.  Ali also develops state and federal reports for the ADRC programs.

Paula Jones

Social Services Case Manager
Paula provides case management services to clients enrolled in the Homecare and Older Americans Act programs.  She conducts assessments for elderly and disabled individuals who are seeking in home support services, ensures access to the continuum of care, ensures appropriate services are provided, ensures the changing needs of the clients and their caregivers are addressed, assures that high quality services are provided in a timely and cost effective manner, and refers clients to appropriate formal/informal agencies to help address any unmet needs.  Paula covers Livingston and Crittenden Counties.

Payton Kidd

Director of Long Term Services and Supports
As Director of Long Term Services and Supports (LTSS), Payton provides strategic leadership and oversight for the Veteran Directed Care (VDC) and Home and Community Based Waiver (HCBW) programs. Payton is responsible for ensuring the effective implementation, compliance, and quality assurance of participant-directed services that promote independence, dignity, and self-determination for older adults and individuals with disabilities. He supervises program staff, reporting requirements, and fosters partnerships with local, state, and federal stakeholders.

Brittany Lyons

Social Services Case Manager
Brittany provides case management services to clients enrolled in the Homecare and Older Americans Act programs.  She conducts assessments for elderly and disabled individuals who are seeking in home support services, ensures access to the continuum of care, ensures appropriate services are provided, ensures the changing needs of the clients and their caregivers are addressed, assures that high quality services are provided in a timely and cost effective manner, and refers clients to appropriate formal/informal agencies to help address any unmet needs.  Brittany covers Muhlenberg and Todd Counties.

Rylee Massie

Social Services Case Manager
Rylee provides case management to clients enrolled in the Participant Directed Services Program/Home and Community Based Waiver. She meets with clients in their homes to conduct program visits and training.  She provides case management on a monthly basis, coordinates services and provides linkage to community resources.

Harley McCarty

Veteran Directed Care Coordinator
As the Veteran Directed Care (VDC) Coordinator, Harley is responsible for managing and coordinating services for eligible Veterans who participate in the Veteran Directed Care program. This position supports Veterans in developing person-centered care plans and ensuring compliance with all VA and agency requirements. The VDC Coordinator serves as the primary liaison between the Veteran, the Veterans Administration Medical Centers, and the spoke agency providers.

Jennifer Medeiros

Home and Community Based Waiver Coordinator
As the Home and Community-Based Waiver (HCBW) Program Coordinator, Jennifer oversees the daily operations and service delivery of Medicaid waiver programs designed to support individuals in their homes and communities. She ensures compliance with federal and state regulations, maintains high-quality services, processes all HCBW applications, and carries a caseload of PDS and traditional HCBW clients. Jennifer provides monthly reporting to management and regulatory agencies.

Heather Meeks

Social Services Case Manager
Heather provides case management services to clients enrolled in the Homecare and Older Americans Act programs.  She conducts assessments for elderly and disabled individuals who are seeking in home support services, ensures access to the continuum of care, ensures appropriate services are provided, ensures the changing needs of the clients and their caregivers are addressed, assures that high quality services are provided in a timely and cost effective manner, and refers clients to appropriate formal/informal agencies to help address any unmet needs.  Heather covers Caldwell and Lyon counties.

Tammy Meredith

Caregiver Programs Support Specialist
Tammy assists in the development, coordination and implementation of programs for Grandparents Raising Grandchildren and services that support caregivers.
This role involves working closely with caregivers, families, healthcare professionals and community organizations to provide resources, support and education to enhance the well being of caregivers. Tammy coordinates and schedules educational workshops, support groups and training sessions. She also provides assessments to determine needs and recommend appropriate resources.

Kayla Milauskas

Social Services Case Manager
Kayla provides case management to clients enrolled in the Participant Directed Services Program/Home and Community Based Waiver. She meets with clients in their homes to conduct program visits and training.  She provides case management on a monthly basis, coordinates services and provides linkage to community resources.

Amanda Monroe

In-Home Support Coordinator
Amanda is responsible for assessing, planning, coordinating, and monitoring services for older adults to help them remain safely and independently in their homes. This role supports aging individuals by linking them to appropriate in-home and community-based services and ensuring care plans are person-centered and responsive to their evolving needs. Amanda ensures daily coordination and oversight of case management and service units, monthly reconciliation of reports, she serves as the backup data administrator and ensures agency compliance with all program regulations.

Marcie O'Neal

Suicide Prevention Task Force Coordinator/Kentucky Suicide Prevention Project
As the Suicide Task Force Coordinator, Marcie plays a critical role in coordinating efforts aimed at prevention and supporting individuals at risk. She is responsible for fostering state and regional partnerships, leading tasking force meetings, providing training, intervention and outreach. She collaborates with grant stakeholders to develop strategic plans and goals for suicide prevention efforts based on community needs and available resources. Marcie assists the AAAIL Director in the collection, analysis of data related to suicide rates, risk factors and prevention efforts.

Amber P'Pool

HCBW Program Support Specialist
As Home and Community Based Waiver Program Support Specialist, Amber provides administrative and programmatic support to the HCBW programs. She plays a key role in ensuring that clients, representatives and stakeholders are supported in accordance with state and federal guidelines. Amber provides administrative support with compliance, service utilization, and ensures efficient scheduling for program related services reviews and program improvement initiatives.

Jessica Robinson

Social Services Case Manager
Jessica provides case management to clients enrolled in the Participant Directed Services Program/Home and Community Based Waiver. She meets with clients in their homes to conduct program visits and training.  She provides case management every month, coordinates services, and provides linkage to community resources.

Amanda Stokes

Director of Aging and Independent Living
As Director of Aging and Independent Living, Amanda is responsible for the strategic leadership, administration, and oversight of programs and services funded under the Older Americans Act (OAA) and other aging-related initiatives. This position ensures the effective planning, coordination, and delivery of services to older adults, caregivers, and persons with disabilities, promoting independence, dignity, and quality of life. Amanda provides supervision to all staff within the programs, serves as data administrator, and oversees the development of the Area Plan on Aging in collaboration with the advisory council and community stakeholders.

Cindy Tabor

Long Term Care Ombudsman
The Ombudsman acts as an advocate for long-term care and group family home residents by investigating and resolving complaints received and assists the Long Term Care Advisory Council.  Cindy also recruits, trains and supervises certified volunteers and friendly visitors, and is the point of contact for the Elder Abuse Council and Forgotten Angels Project.

Stephanie Thompson

Social Services Case Manager / Special Projects Assistant/Drums Alive Instructor
Stephanie provides case management services to clients enrolled in the Homecare and Older Americans Act programs in Christian County. She conducts assessments, home visits, and resource referrals for clients. Stephanie also assists with coordinating special projects and activities for the Area Agency on Aging and Independent Living, along with being a certified Drums Alive Instructor for the Pennyrile region.

Lydia Watkins

Social Services Case Manager / Drums Alive Instructor
Lydia provides case management services to clients enrolled in the Homecare and Older Americans Act programs.  She conducts assessments for elderly and disabled individuals who are seeking in-home support services, ensures access to the continuum of care, ensures appropriate services are provided, ensures the changing needs of the clients and their caregivers are addressed, assures that high-quality services are provided in a timely and cost-effective manner, and refers clients to appropriate formal/informal agencies to help address any unmet needs in Trigg County.  Lydia also serves as a Certified Dementia Care Specialist and as a certified Drums Alive Instructor for the Pennyrile region.

Miranda White

National Family Caregiver Program Coordinator
Miranda is responsible for the day-to-day implementation of the National Family Caregiver Program, including home assessments, case management, and facilitating local Alzheimer’s support groups.  She also assists with the LTC Ombudsman program. Miranda also serves as a Certified Dementia Care Specialist.

Shannon Wynn

Social Services Case Manager/Assessment Specialist/Certified Long Term Care Ombudsman
Shannon conducts comprehensive assessments to identify clients’ needs, strengths, challenges and goals to determine the most suitable supportive services, resources and interventions to effectively meet their needs. Shannon plays a vital role in supporting individuals and families in accessing resources to improve their quality of life.  He also serves as a backup Certified Long Term Care Ombudsman.
