Elderly and disabled residents of the Pennyrile have a host of services available to assist with their in home- supportive and nutritional needs through the efforts of Aging and Disability Services at the Pennyrile Area Development District. Senior centers are currently operating in all nine counties, with supportive services provided there and in clients' homes. Congregate meals are provided at the centers, and for those unable to participate at the site, and meet guidelines, home-delivered meals are distributed. In- home services for individuals with disabilities are also available through the Medicaid HCB Waiver program. Staff is available to meet with clients to assess their needs and determine program eligibility.

Jill is responsible for the overall planning, development and implementation of aging programs and other related services for the Area Agency on Aging. This inlcudes developing the Area Plan, Request for Proposal for service procurement, and contract oversight. Jill also completes administrative monitoring with contractors and provides training to contractor staff. She is available to speak to civic groups and other agencies regarding program services upon request.

Amanda is the point of contact for new client referrals and is responsible for the coordination of Title III, Home Care and Adult Day Care Services. She also provides supervision and training for the case management staff. Amanda provides staffing for the AAA advisory council and monitoring of subcontracts.

Cindy Tabor - Long Term Care Ombudsman (LTC)

The Ombudsman acts as an advocate for long-term care and group family home residents by investigating and resolving complaints received and assists the Long Term Care Advisory Council. Cindy also recruits, trains and supervises certified volunteers and friendly visitors.

James Patterson - Family Caregiver Manager

James is responsible for the day to day implementation of the Family Caregiver Program, including home assessments, case management and facilitating local support groups. He also provides assistance to the LTC Ombudsman program, and assists with contract monitoring of the local senior citizens centers.

Debra helps to coordinate the Kentucky Family Caregiver Program, which provides assistance to low-income grandparents who are raising grandchildren. She also provides Nursing/ICD-9 review for the Consumer Directed Options program.

Payton Kidd -

Payton serves as the lead support broker for
the Consumer Directed Options Program. He screens and processes all
CDO referrals and makes arrangements for initial assessments. Payton
also provides in home case management and training to families enrolled
in the CDO program. Upon request, he is available to provide community
education and training regarding CDO services.

Bruce coordinates the intake/prescreening process through the Aging and Disability Resource Center. He is also responsible for scheduling outreach and enrollment events for the MIPPA program which provides assistance to low income Medicare Part D beneficiaries.

Rachael provides in-home assessment and eligibility services for the Home Care and Administration on Aging programs. She also develops care plans and manages the service waiting lists in the database. Rachael provides Case Management services to clients in Livingston & Lyon counties.

Whitney Freno - Social Services Case Mgr

Whitney provides assessment and enrollment services to clients and families seeking to enroll in the Consumer Directed Options (CDO) program. Whitney meets with clients in their home to conduct the program assessment and establish service eligibility. Whitney provides case management to clients on a monthly basis.
She also provides program training to CDO clients and their caregivers.

Angela Gore - Administrative Assistant

Angela provides administrative support to the Aging staff. She maintains the mailing directory for numerous councils and committees, attends and takes minutes for all aging committee meetings. She also prepares correspondence and is responsible for the daily maintenance of the Home Care and Title III Client charts.

PADD Aging Program Case Managers
Case Managers assess elderly individuals referred by the In-Home Services Manager. Those who are eligible become clients and are scheduled for available services provided by one of the local Senior Centers. Case Managers monitor the client at least monthly to ensure that appropriate services are delivered. Case Managers also provide resource information to clients in an effort to ensure health, safety and welfare.
