2012 Aging Programs - Needs Assessment Survey

This is a comprehensive needs assessment survey created by our agency for the Pennyrile Region, which includes the following counties:
Caldwell, Christian, Crittenden, Hopkins, Lyon, Livingston, Muhlenberg, Todd and Trigg County.

The purpose of the survey is to obtain more information regarding the needs of senior citizens in the region.

The more responses that we receive, the more useful the information.
